We respect and understand that your time is valuable and wish to make the most of your initial visit. Our expert doctors will review your oral scan and cordially address your aesthetic and oral health needs. This is your time. Don’t be shy 🙂
Please assist us by providing the following information at the time of your visit
- Driver’s license or another form of government issued ID
- A list of medications you are presently taking and your primary care physician’s contact information
- If you have dental insurance, please bring your insurance card with you so we can assist in processing your claim
IMPORTANT
All patients under the age of 18 must be accompanied by a parent or legal guardian.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, atrial fibrillation, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medications (i.e., heart medications, aspirin, anticoagulant therapy, osteoporosis medications, immunomodulator therapies etc.).
